Intake and Consent Forms
It is a good idea to print off the necessary forms and fill them out before the initial visit to save time. If you are coming in for an initial visit then the adult intake form and consent forms would need to be completed.
If you are unable to do this ahead of time please come 10-15 minutes early to fill out the paperwork in office.
